The 3.3V line is too high. This drags down the 5v and 12v, as the switching controller tries to compensate.

The the switching controller measures the average error across the 3.3V and 5V line (some 12V as well), and adjusts for that. Bringing the 5V line up would also bring the 3.3V line up, and it's already pretty high...

Purchase a new PSU, or rewind the transformer.
