HTML and web page coding help needed ASAP

Guys I need some help in writing a web page:

I need to create a web page (one single page thats it) which I will host off Geocities or the like which has the following:

A series of text questions written on the page (easy enough, I can do that) with a text box under each to write answers in, not check box, not drop down menu, a text box to type in (this part is crucial LOL, sorry to nag)

With a button at the bottom which says "Submit" and once this button is clicked, will take all the data written into the text boxes and send it to an email address. IE: Mine.

The purpose: I am creating an online survey for my cousin because she needs some of her friends to answer a questionaire which needs to be anonymous, hence the idea of a web page, completely anonymous.

That'll be pretty simple to code, but the only problem is that it requires some PHP to get the email sent. I doubt geocities allows PHP, but if it does I can start looking up a tutorial or just write up a quick script for you.

Edit:
For a textbox in HTML, just add something like:
Quote:
Question1:<br>

<textarea name="question1"></textarea>
The 'name=' will be necessary so php can gather it and send it in the email.

You'd need something more than Geocities. Geocities (even Geocities pro) does not support ASP or PHP. As you can't send an email straight from a webbrowser - you have to have a site you can run the code on.

I used to have a site i coded completely in HTML, was hosted with Freewebs, and i remember i did manage to get it working. You can link to an external script to send the email, there are even services dedicated to this purpose, but i'll be damned if i can remember any.
